Insurance

We are in-network with PacificSource Health Plans, PacificSource Community Solutions, OHP- Open Card, First Choice Health Plans, and Providence Health Plan. With each in-network insurance plan, you are responsible for the copay at the time of service. All other insurance plans are considered out of network. Clients covered by out of network insurance plans qualify for our self pay rate, payable at the time of service. We provide you with a SuperBill so that you may submit for reimbursement from your insurance company based on their out of network coverage rates. 

Rates

Our out-of-pocket rate is $165/50-minute session and $225/90-minute session. 

The out of pocket rate for PhD provider is $215/50- minute session.

We accept cash, check, and all major credit cards.

Cash and check: mail to 686 NW York Bend, OR 97703 or you may drop off the check at reception.

Card: input information through your client portal or call our front desk. Our system will charge your card on the days you have a session.
